In addition to the government assistance participation test above, there are two more requirements for qualifying for Safelink Wireless service.

First, you need to have a valid U.S. street address in order to receive a cell phone through this program. Unfortunately at this point in time, Lifeline Assistance is not available to customers with only a P.O. Box. Are you homeless? If so, you can contact Safelink Wireless and see whether the company has any suggestions for you that might help you navigate around this residency requirement and still qualify for Lifeline Assistance.

The other requirement concerns your household. Only one free government cell phone is allowed per household, which means you can only qualify if nobody else in your household is currently a subscriber to the program (via Safelink or another provider). If someone else already has applied or is receiving Lifeline, you cannot. If nobody else in your household has applied, you should qualify without a problem. Fill it out and attach verification documents that prove you either meet the income test or the qualifying program participation test. You may submit it electronically or send it via fax or snail mail.
