Assurance Wireless currently has a presence in 40 different states as well as the District of Columbia. This makes Assurance Wireless the second largest free government cell phone provider currently operating in the USA. Assurance Wireless is overseen by Virgin Mobile, a Sprint Nextel subsidiary.
If you sign up with Assurance Wireless and your application is approved, you receive a cell phone free of charge and a basic plan of 250 free voice minutes (local or long distance domestic) and unlimited text messages each month. Now for a limited time, new Assurance Wireless customers receive 500 free minutes and unlimited text messages each month for the first four months. After four months the plan reverts back to 250 free voice minutes and unlimited text messages each month. At the moment, Assurance Wireless offers the best plan for residents living outside of Oklahoma (residents of Oklahoma can signup for Yourtel Wireless and get 1,000 voice minutes a month for 1 dollar), with Safelink Wireless coming in a close second.

You will then need to go over the eligibility requirements for your state linked below. If you determine that you are likely to be eligible, then it is time to fill out the Assurance Wireless application and submit it online or through mail or fax. How Do You Qualify for Assurance Wireless Service?
Assurance Wireless continues to expand services, so if you cannot currently receive service through the company, you may be able to soon. You can also see if one of the other free government cell phone providers services your area. If Assurance Wireless does service your state and your local area (double check if you live in an outlying rural area), you are ready to apply. Here are the steps:
Check Eligibility Requirements For Your State
Each state has slightly different eligibility requirements. You will need to look up the requirements for yours. No matter where you live, if you are living at or below 135% of the federal poverty level, you quality. Even if you don’t, you still qualify as long as you are receiving assistance through a federal or state program which qualifies in your state. These programs typically include food stamps, SSI, Section 8, Medicaid, WIC, and so on. Here are the federal poverty guidelines:

For Alaska and Hawaii, the thresholds are higher.
The requirements for eligibility change now and again, which is the other reason you should double check before you apply.
Locate verification paperwork
Regardless of the way you qualify, you will probably need to be ready to provide documentation which verifies your status. That could be a tax return, pay stubs, unemployment stubs, a benefits statement, or so on. When you are checking the requirements, check to see what kind of paperwork you need to provide, and get it together before you apply, since you will need to send it at the same time.
Also note that to apply and get accepted to the Assurance Wireless program, you will need to meet the home address requirement. Unfortunately if you have a P.O. Box, you will be unable to participate in the program. You can only receive your cell phone at a street address. Also make sure nobody else in your household has applied and currently receives Lifeline Assistance service. The government will only approve one free phone per household. Assurance actually asks you to fill in your address as a first step when applying on their site. The company then checks your address and lets you know if you can proceed.

Once you are sure you meet all requirements and you have your verification paperwork ready, you can apply to Assurance Wireless. You can then fill out your application and send it and your verification documents electronically or via fax or mail.
